Are you an individual who will provide safe and therapeutic care
in a comprehensive and systematic manner?The Inova Fairfax Medical
Center - Transplant Pediatric Heart Center is seeking a
self-motivated, high performing Transplant Nutrition Educator with
pediatric experience to develop and manage a world-class
comprehensive nutritional assessment program and care for solid
organ transplant candidates and recipients across the continuum of
care regulatory requirements and professional standards for
nutrition practice.This role maintains an expert level of knowledge
on transplant candidate and recipient assessment and management.
Drives ongoing improvement in transplant clinical practice through
self-development and continuing education. Serves as a content
expert on transplant nutrition assessment and education for the
multidisciplinary team and throughout the organization. The
Transplant Nutrition Educator also utilizes evidence-based
standards and practice for development and implementation of the
transplant nutritional assessment and management program. Develops
and facilitates patient education and support groups for focused
areas of need for each organ program. Applies quality improvement
methodologies to improve overall transplant nutritional assessment
and care services and patient outcomes.This role is a full-time
position with Monday-Friday coverage with no holidays, weekends or
on call.Job Responsibilities:
- Develops patient specific medical nutrition therapy programs
including assessment, instruction, and evaluation of program
effectiveness.
- Provides clinical and educational interventions with
consideration of the cultural and spiritual values and needs of
transplant patients and their families. Interfaces with inpatient
and critical care dietitian team as needed.
- Reviews inpatient team documentation to ensure compliance with
CMS and UNOS regulatory requirements specific to post transplant
care and discharge planning.
- Provides ongoing patient education, nutrition therapy and
counseling to wait-listed patients to ensure ongoing
appropriateness for transplant.
- Collaborates with other specialty dietitians to ensure seamless
provision of excellent care across disciplines (ex. Bariatrics and
diabetes programs).
- Provides transplant specific education annually and as needed
to meet to inpatient dietitian team to ensure competency in care of
organ transplant recipients.
- Provides recommendations regarding pharmacologic and non
pharmacologic therapies.
- Participates fully in the transplant program multidisciplinary
team as the transplant nutrition content expert including patient
selection meetings, quality meetings and daily patient care
/multidisciplinary patient rounds.
- Develops, reviews and revises organ specific transplant
nutritional assessment policies at least annually or as needed to
comply with updates to best practice or regulatory
changes.Qualifications : Experience:
- Four (4) years' experience in transplant services. One (1) or
more years of solid organ transplant nutrition experience and at
least 3 years of acute care clinical dietitian experience or a
minimum of 5 years specialty acute care clinical dietitian
experience.Additional Requirements:
Expertise in organ specific and chronic disease nutrition therapy
such as ESRD, diabetes, heart failure, and chronic lung disease,
Training in functional and frailty assessments, specific knowledge
of food/drug interactions with immunosuppressant
medications.Education:Bachelor's degree in food and nutrition or
dieteticsCertification:
- Registered Dietitian (RD)
- Certified Clinical Transplant Dietitian (certifying body is
NATCO) as a certification to be required within 12 months of start
date in the job
