Edenbridge PACE is opening the first PACE program servicing the
community in Wards 7 and 8 in Washington, DC.Role in Brief:We are
looking for an experienced Certified Nursing Assistant who will be
responsible for providing and assisting participants with
activities of daily living at home, and/or at our Edenbridge PACE
at Town Center. This position reports to the Homecare Coordinator

ReimbursementESSENTIAL DUTIES AND RESPONSIBILITIES Assists
participants with activities of daily living (bathing, dressing,
grooming, hair care, mouth care, eating, hand and foot nail care)
as assigned or needed.Assists in monitoring participant's health
status (vital signs, weight, skin condition, appetite and changes
in functional ability). Observes participant's taking prescribed
medications. Reports pertinent information to Home Care Coordinator
in a timely manner.Documents services provided to participants
consistently and accurately via Electronic Health Record (EHR).
Participates in participant's care planning through cooperation
with multidisciplinary teams. Participates in Quality Improvement
programs as assignedOther duties as requested/assigned professional
demeanor at all times. QUALIFICATIONS Must hold active
certification by Washington DCMust have 1 year of experience with
the frail, elderly or disabled population. Must be 21 years of age
or older.Must have good written and verbal communication skills.
Must have a current Driver's License and driving record must be in
compliance with Company policy at all times. Must maintain current
automobile insurance. Must submit to and pass a pre-employment and
criminal background check. Must be free of all communicable
diseases and have all immunizations up to date before providing
direct care. Vaccination against the seasonal flu and Covid - 19
required. Must be efficient with computer applications such as
email, navigating the internet, using electronic timekeeping, and
Electronic Health Records. Operates in full compliance with all
applicable federal, state, and local fire, health, safety,
sanitation, Emergency procedures, and other Standards prescribed in
law or regulationsAbility to work independently with minimum
supervision. Knowledge of how to use and maintain in-home equipment
