Under the supervision of the Clinical Director, the RN Field
Nurse will handle the assessing, planning, organizing,
administering, documenting, and supervising client care in
accordance with the principles and practices of home care nursing.
Specifically, this position will conduct the initial assessment and
ongoing reassessments to develop and implement an individualized
care plan for each client including recommendations for appropriate
services. This position will communicate with the physicians as
needed to obtain orders and updates with the client status. This
position administers medications and treatments to clients, and
occasionally oversees the work of the LPNs, CNAs and CMTs, as
designated. All tasks and functions performed by this position will
be carried out consistent with the Agency's policies and
procedures, and quality standards. Responsibilities: Clinical:
Performs the initial assessment and ongoing reassessments to
evaluate the client and determines the appropriate services for the
client. Communicates with the physician as needed to obtain orders
and updates on client status. Develops and implements an
individualized care plan for each client based on the assessment
and in conjunction with the client, family, and physician and/or
authorized practitioner. Delivers skilled nursing care as outlined
in the plan of care prescribed (when required) by the physician
and/or other authorized practitioner according to acceptable
nursing standards. Observes signs, symptoms, and changes in the
clients physical or emotional condition. Consults with the
physician or authorized practitioner concerning alteration of the
plan of treatment in consultation with the supervisor, as
necessary. Evaluates the effectiveness of his/her nursing service
to the individual family. Teaches, supervises, and counsels the
client/family and/or caregiver regarding the nursing care need and
other related problems of the client at home. Records clients
condition, interventions, and evaluation in the medical record as
documentation of home visits according to agency policy. Enters
visits/time in ADP weekly for payroll processing if applicable.
Enter time/tasks via telephony for each client visit if applicable.
Participates in case management conferences and other
client-related meetings with team members and other health care
providers as appropriate. Coordinating: Provides direct nursing
care as ordered by the physician or authorized practitioner.
Collaborates with other professionals involved with skilled care
cases to coordinate other ancillary services (e.g. durable medical
equipment, infusion therapy, etc.) for clients as appropriate.
Performs joint on-site visits with Care Coordinators and Client
Services Managers when appropriate. Managerial: Trains and orients
other home care nurses as needed. May indirectly supervise LPNs,
CNAs CMTs. On-Call: Participates in on-call rotation as needed.
Requirements: 3-5 years of experience in home care, hospice,
Medicare home health and/or in a private duty home care setting.
3-5 years of clinical nursing experience with experience performing
patient assessments preferred. Broad knowledge of the theory and
practice of home health care, nursing processes, and nursing
practice. Willingness and desire to work with the seniors and
knowledge of the aging process, physical, psychological, and social
needs of older adults. Ability to effectively communicate with a
variety of people. Ability to direct client care and services to
older adults which meet or exceed the Agencys quality and service
standards. Excellent time management skills. Demonstrated ability
to perform physical and psychosocial assessment of client and
family. Demonstrates effective customer service skills and caring
attitude. Demonstrates skill in conflict resolution. Ability to
effectively represent the Agency in the community-at-large.
Familiarity and understanding of resources for seniors is a plus.
Flexibility to adapt to different business expectations,
challenges, and goals. Goal and detail oriented; able to work
independently and in a tight team setting. Skilled and effective
telephone demeanor and customer service skills to develop a rapport
and confidence in the companys services with all callers, clients,
and industry professionals. Excellent organizational skills,
attention to detail, ability to multi-task, and work well under
pressure despite numerous interruptions and/or background noise.
Demonstrated critical thinking and analytical skills. Currently
licensed to practice nursing in MD and DC, or eligible for DC
license. RN required, BSN preferred.
