Summary: The Registered Nurse (RN) Assessment Nurse is responsible
for conducting comprehensive in-home clinical assessments and
evaluations for clients receiving home care services. This role
focuses on determining care needs, developing initial care plans,
completing reassessments, and ensuring compliance with state
regulations and agency standards. The RN collaborates with the care
coordination team to support safe, high-quality services while
maintaining accurate and timely clinical documentation. The RN
Assessment Nurse does not provide ongoing shift-based nursing care
but serves as a clinical evaluator to support intake, care
planning, and quality oversight. General Accountabilities: Conduct
in-home assessments, reassessments, and clinical evaluations to
determine client eligibility and level of care needs. Develop and
update individualized care plans based on assessment findings and
physician orders when applicable. Ensure all assessments and
documentation meet Maryland state regulations, Medicaid standards,
and agency policies. Communicate assessment outcomes and
recommendations to the care coordination team, office staff, and
caregivers. Identify safety risks, changes in condition, and
potential barriers to care, escalating concerns appropriately.
Maintain accurate, timely, and compliant clinical documentation
within the agency’s electronic system. Provide clinical guidance
and recommendations to support quality home care services.
Participate in case reviews, quality assurance activities, and
compliance initiatives as requested. Required Qualifications:
Maintain active RN licensure, required certifications, and ongoing
professional competence. Represent the agency professionally during
client interactions, maintaining confidentiality and HIPAA
compliance. Travel to client homes or designated locations to
complete assessments within assigned service areas. Active and
unrestricted Registered Nurse (RN) license in the State of
Maryland. Minimum of one (1) year of clinical nursing experience;
home care, community health, or case management experience
preferred.
