positively impacting the community! Responsibilities About the Role
During visits that may last up to one hour, our providers assess a
member’s current health status, medical history, medication
adherence, social circumstances, and other potential risks. This
approach provides invaluable insights into a member’s overall
health that are often missed during standard office visits. The
Matrix Comprehensive Health Assessment (CHA) enhances care quality
and allows us to address multiple care gaps in a single visit. Our
providers: Perform assessments for adults and geriatrics, including
medical history, diagnosis, treatment, health education, physician
referrals, case management referrals, follow-up, and thorough
documentation in line with our guidelines and protocols. Administer
and order necessary point-of-care and lab tests. Collaborate with
physicians, case managers, social workers, family members, key
caregivers, and other medical personnel as appropriate. Work
alongside Primary Care Physicians (PCPs) for patient education and
follow-up as needed. Provide services at the member’s residence,
whether in their homes or Skilled Nursing Facilities. Benefits of
Joining Our Team Consistency – While assessments are conducted from
Sunday to Saturday between 8 AM and 7 PM, our full-time providers
typically work Monday to Friday from 8:00 AM to 5:30 PM.
Competitive Pay – Enjoy an annual base salary ranging from $105,500
to $168,400 , determined by market analysis and experience.

Additional Licensure – If business needs arise, we will assist you
in obtaining additional state licensure and credentialing in
neighboring states or others you may wish to visit while conducting
health assessments. Qualifications Essential Skills & Experience A
Master’s Degree or equivalent experience with successful completion
of NP or PA licensure. Current NP/PA licensure in the state of
practice, including prescription authority or the ability to obtain
it as required. NCCPA, AANP, AACN, or ANCC board certification in
Adult, Adult Gerontology Primary Care, Adult Gerontology Acute
Care, Family, Emergency, or Acute Care specialty. Current BLS,
ACLS, or CPR certification. A minimum of one year of NP or PA
experience. Willingness to travel 80-90% of the time, including
overnight and weekend travel across the assigned state and possibly
neighboring states. A valid state driver's license and proof of
adequate automobile insurance for the state of residence.
Proficient computer skills and familiarity with health/medical
record software. Excellent verbal and written communication
