Job Description:
*Clinical Magnet (part of ICON Medical)* is seeking a dedicated
and compassionate *Licensed Practical Nurse (LPN)* to join our
partners *Home Care team* in *Charlottesville, VA*.This is a
*full-time, direct hire* opportunity with a supportive team and a
meaningful mission: delivering care to patients in their
homes.*Position Highlights:** *Schedule:* MondayFriday, 8:00 AM5:00
PM (with weekend rotation)* *Compensation:* $28.85$36.06/hr, based
on experience* *Start Date:* ASAP* *Employment Type:* Full-time,
Direct Hire* *Location:* Charlottesville, VA (must have reliable
transportation)* *Reports to:* Kim (Recruitment Contact)*Key
Responsibilities:** Deliver general nursing care, including
administering medications and treatments per physician orders*
Provide in-home services and follow assigned care plans in
compliance with regulations and agency policy* Conduct diagnostic
and preventative procedures, including phlebotomy and vaccinations*
Educate participants and families on medications, treatments, and
care instructions* Maintain thorough and accurate documentation of
all care provided* Monitor patient responses and report conditions
or changes to physicians and RNs* Coordinate with interdisciplinary
teams to implement and adjust care plans* Participate in quality
assurance activities, peer reviews, and care planning meetings*
Supervise and collaborate with healthcare personnel including CNAs*
Perform duties both at client homes and within agency facilities as
needed*Requirements:** *Active LPN License* in the state of
Virginia* *Current First Aid and BLS Certification* (American Heart
Association or Red Cross accepted)* *Valid drivers license*,
personal transportation, and auto insurance* *High School Diploma
or GED**Preferred Qualifications:** 3+ years of LPN experience in a
clinic or long-term care setting* Experience working with geriatric
patients* Familiarity with medications commonly used in elderly
