  Key Responsibilities:
  
  
  • 
    
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
    
  • Observe, record, and report patient reactions, changes, and progress to the attending physician and any appropriate agency staff.
    
  • Treat patients to relieve pain, develop or restore function, and maintain maximum performance using physical means.
    
  • Minimize patient risk by front-loading visits for high-risk patients and teaching them preventative measures.
    
  • Counsel patients and family members on disease/injury management and proper use of equipment.
    
  • Supervise therapy services provided by PTAs and nurse assistants.
    
  • Communicate with PTAs and nurse aides on supervisory visit results.
    
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
    
  • Understand and follow agency policies, procedures, rules, and regulations.
    
    
    Requirements:
    
    
    • 
      
    • Physical Therapist with a current license in the state of employment.
      
    • Minimum of one year of experience as an PT in an acute setting.
      
    • Valid driver's license and auto insurance in your name as a driver.
      
    • Capable of all physical demands.
